Female Hormone Panel


Very painful menses.

This patient was experiencing quite a bit of pain during menses. The doctor ordered the Pre Menopause Female Hormone Panel (FHP) so we could see what her hormone levels were throughout the whole cycle.

The shape of her estradiol curve was actually not too bad. However, it was a little high overall setting her up for some symptoms of estrogen dominance if the progesterone was inadequate. Progesterone output actually was more than adequate for a few days but fell dramatically several days before the end of the cycle. In my opinion, that drop was the problem. We call it a Type III Luteal Phase Deficit (or Defect) which really means that progesterone was not distributed well across the whole luteal phase (ovulation to her next period).

The patient's testosterone was normal, but her DHEA was low indicting she was under some stress.

I think the patient would benefit from taking sublingual progesterone to help correct the rapid drop in progesterone the last few days of the luteal phase. Also, the doctor might consider using the Adrenal Stress Index (ASI) to measure adrenal function and liver support to help clear the slightly high estradiol.
